WE ARE CREDENTIALED WITH Anthem Blue Cross Blue Shield and United Health Care. COVERAGE DEPENDS ON INDIVIDUAL BENEFITS.
Fee for Service Notice for Nutrition Counseling
In-network Insurance: Katie Hake Health & Fitness, LLC currently accepts Anthem Blue Cross Blue Shield and United Healthcare. Insurance is billed directly on your behalf, if your coverage has been verified in advance of your visit. If insurance rejects a submitted claim for any reason, the client is responsible and will pay the full fee for the service(s) rendered.
Out-of-network Insurance: If we are not in-network with your insurance, your clinician can provide a superbill that you can submit directly to your insurance company. If a superbill is requested, clients are responsible for privately paying at the time of the service. Additionally, submitting a superbill does not guarantee reimbursement to cover the services provided.
Out of Pocket Fees: If not covered by or not using insurance, out of pocket fees are $240 for initial sessions and $150 for follow-up sessions per 60 minutes of service. This is separate from our Reboot Bootcamp Program.
Medicaid/Medicare: We are not in-network with Medicaid or Medicare at this time. If you have one of these plans, we encourage you to reach out to your insurance provider for a list of in-network providers.
It is the responsibility of the client to verify whether their insurance plan covers nutrition counseling services. Clients should review their policy documents and contact their insurance provider directly to confirm coverage details, including any limitations or requirements such as referrals or pre-authorizations.
